1. Sign up today. Complete the attached enrollment form, bring it to one of our offices and we’ll enroll you in the program. (If you’ve received this brochure along with a “thank you” letter for a referral, you’ve automatically been enrolled and can skip this step.)
2. Share your story with people you know who may be struggling because of an injury or chronic, painful condition.
3. Refer someone by giving them the attached “Ticket to Health,” which is good at all ProFitness office locations. Be sure to write your name on the ticket so we can properly thank you.
4. Earn rewards when people you’ve referred get the help they need and start treatment at ProFitness Physical Therapy.
